This says that the two girls were waiting for their dad to finish his work before going to a doctors appointment 🙁.


Thats just so hard to wrap my head around. One of my favorite things is when my kids come visit at work. I can't imagine my wife dropping em off just like any other day and then having to at some point realize they're never going to make it back home.
 
Throughout the facility, the employer is failing to establish and maintain an adequate energy control program.
The employer is failing to ensure that all employees operating powered industrial trucks at the facility are trained in accordance with this subpart in that employee(s) are not receiving training on the operation of the Caterpillar fork lift Model FG25
Cleanliness around working machinery was not maintained to prevent fire hazards: In the Mill and Basement area of the facility, the employer is failing to ensure that wood dust does not accumulate
Throughout the facility, the employer is failing to develop, implement and/or maintain a written hazard communication program as outlined in this subpart.
At the facility, the employer is failing to protect employees from potential chemical exposure hazards associated with not conducting required HAZCOM training
Points notable to me in bold.
I Deal with OSHA on a regular basis, they would have been reinspected multiple times since that initial inspection in 2012 and been checked for all those things again, since they have not had repeat or willful violations those issues would have been rectified and signed off by an osha inspector more than likely about 3-5 times since then.

that said just because the rectified those issues doesn’t mean there were others that OSHA didn’t inspect. There is likely some employer fault that led to the tragedy
